2008-09 Central Arizona Region EMS & Trauma Needs Assessment

This year is the first year that AEMS is offering the EMS and Trauma Needs Assessment to be completed electronically.

If you do not have access to the web, we will provide you with – UPON REQUEST - the needs assessment as a hard copy for you to complete and return via fax or regular mail.

The above links will automatically open to the EMS and Trauma Needs Assessment. We believe that you will find the completion process self-explanatory and easy. IMPORTANT: you must complete all questions marked with an * (asterisk). At the end of the needs assessment, you must click “DONE” to submit. The Needs Assessment contains FIVE PAGES of information queries to complete.

Finally, please note that both the 2008 EMS and Trauma Needs Assessment is due before 5:00 pm on May 5, Monday.

2008-09 Central Arizona Provider Assistance Application Process for Rural EMS Providers

This year is the first year that AEMS is offering the Provider Assistance Application and EMS and Trauma Needs Assessment to be completed electronically. While the application has been tested, there may be glitches we are unaware of. If you should encounter any problems completing the application, please let us know ASAP.
